At Prague Medical Holiday we believe that our potential clients before asking us to act on their behalf want to understand and know about:
The price which we negotiate with the provider hospital/clinic will not include:
2 (d) We will provide advice on Visa requirements and obtain for you the relevant documentary evidence regarding the arrangements made for you to receive medical treatment to facilitate entry into either Czesh Republic at the port of entry.
2 (e) In the process of sourcing and negotiating treatment on your behalf, Prague Medical Holiday will disclose the personal and medical history information to hospitals and clinics. We will take all reasonable care to ensure that these hospitals/clinics are bound by confidentiality and privacy obligations in regards to protecting your personal information. Furthermore this sensitive information will be used or disclosed only for the purpose for which it was given. We have also implemented all reasonable precautions to ensure that your personally identifiable information is retained in a secure fashion and protected against loss, misuse and unauthorized access, disclosure, alteration or destruction.
2 (f) We will provide information on the availability of specialized Medical Tourist Travel Insurance. We will provide advice you how to obtain this insurance.
Prague Medical Holiday strongly advises that you obtain this insurance.
3 (a) It is important that you provide timely and accurate information in order for us to deliver the service to you. Failure to provide the information may result in Prague Medical Holiday being delayed or unable to provide the services to you. Prague Medical Holiday takes extensive precautions to ensure that the personal information we collect, use and or disclose is complete and correct. However, the accuracy of the information is subject largely to the details you submit. With this in mind Prague Medical Holiday would ask you to advise us if there are any omissions or errors in your personal details and of any changes to your personal circumstances or contact particulars, (name/address) as soon as possible. Prague Medical Holiday cannot be held responsible for delays or refusal to treat on arrival in Czech Republic due to the non-declaration of material facts relating to your health. Completion of our Medical History Questionnaire and obtaining a Release to Travel Letter from your Family Physician is designed to prevent this happening.
3 (b) We ask that prompt payment of the initial Deposit and the Final Fee are made within 3 working days. This will prevent delay and in relation to the Final Fee the losing of a treatment slot. The amount can be paid by PayPal or electronic bank transfer. You are responsible for paying the fixed - all inclusive fee of the Hospital/Clinic at which you are to receive treatment. In addition you are responsible for paying the nursing agency for any follow-up care which we arrange on your behalf.
3 (c) We request that all our clients take out a specialized Medical Tourist Travel Insurance policy. We can refer clients to the appropriate brokers.
3 (d) Clients take responsibility for visa application and cost.
